To expedite your first visit we have our New Patient form available on-line. After you have made your first appointment you may fill it out and e-mail it to us (or print and mail it to us). If you have not made your first appointment, please call us at 480-899-4477 and we'll get you scheduled.

Kelly Jorn Cook, DDS accepts most dental insurance plans. For your convenience we offer two in-house financing programs through Care Credit and Chase Health Advance.
